Position Description
Overview
The Nurse Practitioner reports to the Chief Medical Officer, CINQCARE., with accountability for providing clinical judgment, organization, and evidenced-based analysis in keeping with CINQCARE @ Home model of care. The Nurse Practitioner should embody CINQCARE’s core values, including Imagination, Trusted, Empathetic, Committed, Creative, Humble and Community-Minded. Primary Responsibilities:
The Nurse Practitioner will have the following responsibilities:
- Provide general medical care to various and/or specific patient levels – adults, geriatrics, and pediatrics.
- Perform comprehensive evaluations including history and physical exams for risk adjustment and other regulatory assessments.
- Address both chronic and acute primary care complaints, and able to ascertain medical urgency.
- Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom; able to work within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately.
- Create and implement a medical plan of care.
- Provides care coordination and preventative care services for geographical as well as virtual targets who are stratified by risk.
- Provide post discharge coordination to reduce hospital readmission rates and emergency room utilization.
- Perform face-to-face in-person visits in a variety of settings.
- Additionally, perform face-to-face synchronous video communications using Tele-Health platforms based on business need and leadership direction.
- Prescribe medications as appropriate.
- Perform timely documentation in an electronic medical record computer system or other method of documentation.
- Engage in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws.
- Perform other job-related duties as assigned.
General Duties
The NP will have the following duties:
- Leadership: The NP will lead in defining and executing strategies and solutions to create business value for CINQCARE.
- Strategy: The NP will establish the business strategy and roadmap: improve outcomes for CINQCARE’s Family Members; enhance the efficacy of the practice. In establishing the business strategy, the NP will define and innovate sustainable revenue models to drive profitability of the Company.
- Collaboration: The NP will ensure that CINQCARE’s capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the clinical objectives of CINQCARE @ Home model of care.
- Knowledge: The Nurse Practitioner will provide clinical subject matter expertise, seek specialty consultation as appropriate and is responsible for knowing when a patient needs physician oversight and when the patient's medical condition may be beyond the scope of knowledge of the NP.

Qualifications
The Nurse Practitioner should have the following qualifications:
- Education:
- Nursing Degree or Nurse Practitioner Degree from an accredited U.S. school is required.
- Nurse practitioners must have completed accredited education and training for nursing and nurse practitioners.
- An active unrestricted medical license in the State of New York is required.
- DEA Certification required.
- AANP Certification required
- ANCC Certification required
- Experience: At least 3 years of active practice required. DEA Certification required. Preferred experience in Health Home programs as a licensed clinician, especially in management of chronic conditions. Fluency in a language in addition to English is a plus. Immunization and phlebotomy skills.
- Entrepreneurial: CINQCARE seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. The Nurse Practitioner will possess the ability to perform certain in-office procedures safely in an outpatient setting.

Compensation: $110,000-119,000
