SUMMARY/OBJECTIVE:
The Clinician- Team Lead is responsible for maintaining a client caseload in which they identify goals and objectives to help improve emotional, social and daily functioning. This position is also responsible for identifying the treatment needs of their supervisee’s(the clinic’s team leads) clients and making sure they are being met in accordance with agency policies, procedures, and state and federal guidelines and regulations. They are responsible for managing the successful execution of the Agency’s policies, procedures, practices, standards and program-based systems within their clinical team.
Essential Functions (Job Duties)
1. Complete a comprehensive biopsychosocial history assessment that includes the resident's problems and strengths and preferences, including cultural, religious, and ethnic background.
2. Responsible for the provision of admit notes that clearly articulates medical necessity for outpatient mental health, substance abuse, or co-occurring treatment. This occurs by the third face to face visit, 30th day from 1st contact, or when prescribed medication- whichever comes first.
3. Responsible for the initiation of a comprehensive treatment plan by the 4th face to face visit or by 30 days from admission- whichever is first.
4. Responsible for assisting clients in creating and maintaining treatment plans every 90 days.
5. Orients residents and families to the facility, its services, its service limitations and residents' rights.
6. Involves residents and families in care planning, encouraging their attendance at care conferences.
7. Completes mandatory documentation on EHR within timelines established by applicable regulations.
8. Empowers clients and encourages client’s choice in matters affecting them, demonstrating receptive attitudes and sensitivity to choices based on racial, cultural, and ethnic heritage, and sexual orientation.
9. Provides linkage with appropriate community resources by maintaining knowledge of other systems, making referrals and identifying unmet needs (e.g., recreational transportation, adaptive phone equipment, etc.)
10. Provides clients and families with crisis management services.
11. Develops a comprehensive discharge plan in collaboration with clients, clients’ families, multidisciplinary staff, and referral agencies.
12. Provide individual, group and/or family counseling for mental health and/or substance use.
13. Ability to collaborate with other local agencies (DSS, Probation, Drug Court).
14. Complete urine drug screens on clients.
15. Accommodate any walk-in assessments and/or crisis’s during business hours, if schedule allows.
16. Monitoring supervisee’s adherence to OMH/OASAS regulations and procedures; and ensuring compliance with HIPAA policies and procedures.
17. Train clinical staff as needed in EMR, required paperwork/documentation, etc.
18. Provide clinical supervision to supervisees on a weekly/bi-weekly basis (based on licensure requirements, needs, and experience), when licensed. Clinical supervision should include activities such as case review and discussion of treatment plans. There should be one-hour group meetings weekly to perform these duties.
19. Provide coverage for cases when supervisees are on vacation or otherwise unavailable on short notice
20. Create and implement goals for each supervisee.
21. Performs all other duties as necessary for the good of the clinic/agency, as instructed by the Clinic Manager.

Position Type/Expected Hours of Work
This is a full-time time position. Workweek schedule: Monday through Friday. Clinic is open from 8am-5pm Monday, Tuesday, Wednesday, and Friday and from 8am-7pm on Thursday. Employees work a 40-hour work week. Hours and schedules may vary depending on Company need and workload. Employees will be given as much advance notice as possible when such changes occur. This position requires a minimum of 2 day sof on-site work per work week.
Travel
Occasional client home visits.
Required Education and Experience
1. Master’s degree in social work or mental health from an accredited school.
2. Licensed eligible.
3. Experience working with mental health and substance use population.
4. Supervisory experience is preferred.
