Citizens Memorial Hospital (CMH) is recruiting a Licensed Practical Nurse (LPN) to assist and triage patients over the phone when calling their primary care clinic.
Care Transitions answers and returns patient phone calls for CMH primary care clinics. The department is made up of two teams, schedulers and triage nurses, who work closely together.
A Typical day will include: Answers and returns patient calls for triage and nursing advice. Relays information and results between patients and their primary care provider. Assists with facilitating medication refills, authorizations and questions. Identifies and connects patients to organizational resources. Provides excellent customer service and behaves in a manner consistent with the mission of CMH. Performs other duties as requested.
The position is: Full-Time, Monday - Friday 8:00 a.m. - 4:30 p.m. No weekends, no on-call, or holidays.
