Position Summary
CityServe Network is seeking a compassionate, organized, and experienced Enhanced Care Manager (ECM) Lead Care Manager to support high-need members through California’s CalAIM ECM program. The ECM Lead Care Manager provides leadership, oversight, and direct care coordination to ensure members receive person-centered, trauma-informed services that address medical, behavioral health, and social needs.
Key Responsibilities
- Support ECM Care Managers to ensure high-quality service delivery and compliance with CalAIM ECM requirements
- Provide direct care management for a caseload of high-acuity members when needed
- Conduct comprehensive assessments, develop individualized care plans, and coordinate services across medical, behavioral health, housing, and social service systems
- Serve as a primary point of contact for community partners, clients, health plans, providers, and internal teams
- Monitor documentation, billing, and reporting to ensure accuracy and timeliness
- Participate in case conferences, multidisciplinary team meetings, and quality improvement initiatives
- Ensure services are delivered using trauma-informed, culturally responsive, and member-centered practices
- Support onboarding, training, and mentoring of ECM staff
- Maintain compliance with HIPAA, organizational policies, and program guidelines
Required Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, Public Health, or a related field (or equivalent experience)
- Minimum of 2 years of care management, case management, or social services experience (ECM or high-acuity populations preferred)
- Demonstrated leadership or supervisory experience
- Knowledge of CalAIM ECM, Medi-Cal, and community-based resources preferred
- Strong organizational, communication, and documentation skills
- Ability to work independently and as part of a multidisciplinary team
- Reliable transportation required
- Valid California Driver License required
Work Environment & Physical Requirements
- Community-based work with regular travel throughout Bakersfield and surrounding areas
- Ability to sit, stand, walk, and drive for extended periods
- Occasional lifting of light materials (up to 25 lbs)
