Job Summary
The Healthcare Home Coordinator (HCH) at Clark Community Mental Health Center provides essential administrative, organizational, and care coordination support for clients enrolled in the HCH Program. This role emphasizes tracking health metrics, providing clerical support to the care team, and facilitating wellness-related events. The HCH Coordinator helps to ensure clients receive integrated care and remain engaged in their health and recovery journey.
Duties and responsibilities
Care Coordination and Monitoring:
- Track client hospitalizations, including admissions and discharges, and coordinate hospital follow-up appointments.
- Communicate with healthcare providers to gather relevant documentation from hospital stays (e.g., discharge summaries, medication lists).
- Track and coordinate with case workers to facilitate the completion of metabolic screenings for clients engaged in Community Support programs.
- Track and coordinate health measures as defined by state reporting and data measures and outcomes.
- Occasional assistance with transportation and conducting in-home visits.
Clerical and Administrative Support:
- Provide clerical support to the HCH team, including scheduling appointments and preparing correspondence.
- Maintain accurate and up-to-date records of all client interactions and services provided.
- Assist in the preparation of reports and data entry related to the HCH Program’s performance metrics and outcomes.
Client and Provider Communication:
- Ensure timely communication between clients and healthcare providers, facilitating the exchange of information between mental health and physical health professionals.
- Assist clients with understanding their care plans, hospital discharge instructions, and follow-up care needs, when appropriate and in consultation with nursing staff.
- Attend multidisciplinary team meetings.
Event Facilitation and Health Promotion:
- Organize and facilitate health promotion events such as wellness screenings, health fairs, and educational events for clients.
- Encourage client participation in events that promote healthier lifestyles, such as smoking cessation, weight management, and exercise programs.
Documentation and Compliance:
- Assist assigned HCH Nurse with all care coordination activities and hospitalizations in accordance with Medicaid and other state/federal requirements.
- Ensure compliance with HCH policies, standards, and regulations by maintaining accurate client documentation and participating in audits.
Team Collaboration:
- Work closely with the Healthcare Home Nurses, Care Managers, and Behavioral Health Providers to support integrated care delivery.
- Participate in regular multidisciplinary team meetings to discuss client progress and review hospitalization cases.
- Support the team in meeting program goals and fulfilling Healthcare Home requirements for clients with complex health and behavioral health needs.
Qualifications
- Education: High School Diploma or GED required.
- Experience: 1-2 years of experience in a healthcare or mental health setting, preferably in an administrative or care coordination role.
- Skills:
- Strong organizational and clerical skills, including data entry.
- Ability to track and monitor hospitalization records and ensure follow-up care.
- Excellent communication and interpersonal skills to facilitate events and interact with diverse populations.
- Proficiency with electronic health records (EHR) and Microsoft Office.
Work Environment
The HCH Coordinator works in an office setting but may occasionally need to attend community events or meetings at other healthcare facilities. Occasional travel to client homes can be expected.
