Description
The individual will engage families who have received a Family Assessment Response from DCF. The Parent Navigator will work as part of the team which includes the family, Community Support Worker and other involved supports to facilitate the engagement of the family. The Parent Navigator will connect the family to concrete, traditional and non-traditional supports within the community.
Key Responsibilities:
- Working collaboratively with the Community Support Worker, family and involved supports
- Utilizing the wraparound family team model
- Traveling within the Greater New Haven service area to provide services
- Partnering with the family and other supports, to develop a strengths-based, family-driven plan of care
- Participating in child and family team meetings
- Advocating, empowering and mentoring families to take the lead in their own plan of care
- Documenting all contacts with families/ supports
- Other duties as assigned by Supervision
Qualifications
- Valid driver’s license and reliable transportation
- High Diploma
- Preferred minimum of 3 years experience with families in need
- Ability to work as part of a team
- Organization skills
- Strong communication skills
- This role may require travel throughout the Greater New Haven area
