CSO is looking for a Family Partner for our CBHC Clinic in Athol.

Community Behavioral Health Centers (CBHCs) are responsible for providing a full range of client-centered and family-centered services and interventions for mental health, substance use disorders, and co-occurring disorders. CBHCs serve as an entry point for timely, flexible, person-centered, high-quality mental health and addiction treatment on an urgent and ongoing basis.

Offer support and understanding to children and families in crisis by helping parents/caregivers gain the skills, self-confidence and access services to meet their child's complex needs.

Responsibilities

  • Work in conjunction with CBHC ESP Clinician and other staff to complete assessments and Family Support Plans (including CANS Assessment when needed).
  • Provide support for parent and/or family members through process of assessment and Family Support Plan development; encourage involvement of natural supports for families.
  • Oversee implementation of the Family Support Plan.
  • Provide direct supports to family members.
  • Deliver services on-site or in the parent/caregiver's home or community.
  • When applicable, provide education, coaching, training and assistance to parents in navigating child serving systems (DCF, DMH, juvenile justice).
  • For families engaging in Intensive Care Coordination services, participate in Wraparound processes including completing paperwork, attending Care Plan team meetings, and communicating with Care Coordinator.
  • As a designated critical employee of CSO, must come to work when scheduled regardless of adverse weather conditions, and when other CSO offices are closed, including holidays. In the event there are extenuating circumstances, employee must make prior arrangements with Program Director in order to maintain adequate staffing in the program.
  • Complete all initial and ongoing credentialing requirements for those hospitals where employee will be providing services including obtaining required vaccinations, tox screens, and safety/health education competencies. (Any necessary vaccinations/x-rays are employee's responsibility.)
  • Foster empowerment and building self-efficacy.
  • Facilitate linkages to peer/parent support and self-help groups as necessary.
  • Attend in-service trainings on evidence-based best practices and other topics as appropriate.
  • Ensure record keeping and service delivery standards meet CBHC quality metric requirements, as determined by the CBHC contract.
  • Complete required documentation in a timely manner.
  • Assist in identifying formal, natural support and community resources.

Qualifications

Minimum high school diploma or GED and two years' experience working with children, adolescents or transition age youth. Experience as parent/caregiver of a youth with special needs, and preferably a youth with mental health needs; willingness to share this experience/journey with child serving systems. Must be able to work effectively within the mental health system, with team members, and effectively empower parents/caregivers. Will be able to document activities in a manner that meets agency requirements. Willing to participate in job-related training when required. Ability to occasionally work evenings and weekends with some flexible hours. Ability to work in unpredictable environments, occasional chaotic social settings, and with clients of varying cultural backgrounds. Computer proficiency required including ability to work with email, electronic health record and word processing software. Valid driver's license and reliable transportation required to support outreach and home and community-based activities.
