What are the responsibilities and job description for the Shelter Community Support Provider position at Clinical & Support Options?
CSO is looking for a Shelter Community Support Provider to work with shelter guests in Greenfield.
The Shelter Community Support Provider will provide intensive, clinical case management to consumers with serious mental illness needing such support, or who have been hard to engage in the mental health system. Services will be provided at the Shelter or in an outreach setting. The Shelter Community Support Provider will have regular contact with consumers, and will provide support services for family members and others in the "natural helping network."
As a Shelter Community Support Provider:
You Will:
- Provide skills training and assistance in areas of daily living activities including transportation, money management, cleaning, laundry, food preparation, personal grooming, and health care
- Assist with advocacy needs; facilitate "mediation" as appropriate
- Provide assistance in identifying and obtaining entitlements, which meet clients needs
- Assist clients with identifying appropriate mental health and medical services and accessing services of choice
- Provide assistance with developing and maintaining a "natural support network" of family and friends
- Assist clients in maintaining the appropriate upkeep of apartment or shelter space that will ensure healthful and safe living
- Assist client in negotiating relationships with others
Your Background:
- Bachelor's or Associate's degree in a mental health discipline OR high school diploma and five years' related experience
- Experience working with at-risk populations
- Valid driver's license and reliable transportation
