Role Description
We are seeking a Clinical Care Coordinator for a full-time, on-site role in Boulder, CO. The Clinical Care Coordinator will be responsible for coordinating patient care, maintaining patient records, and supporting clinical staff in delivering personalized, holistic healthcare. Additional responsibilities include assisting with scheduling, managing communication between providers and patients, and ensuring that operational processes align with the clinic's quality and service standards. This role requires strong organizational skills, a friendly and professional demeanor, and the ability to work closely with a multidisciplinary team to support optimal patient outcomes.
Role Responsibilities
- Manage patient clinical communications via patient portal, Spruce, phone, and other channels
- Ensure smooth transitions between clinical and administrative workflows
- Triage inbound messages and escalate urgent or complex concerns to providers
- Perform clinical callbacks for diagnostic result notifications and next-step explanations
- Sort and organize incoming clinical results for daily provider meetings
- Update patient records timely and accurately
- Proactively identify overdue or abnormal results and escalate as needed
- Support medication management
- Partner with providers and care team members to ensure continuity of care
- Provide standard nursing visits, injections, phlebotomy, and other clinical support as backup
- Assist providers and clinical staff during peak times or staffing gaps
Qualifications
- Required: Registered Nurse (RN)
- Strong organizational and administrative skills, including patient scheduling and record management
- Exceptional interpersonal, communication, and customer service skills
- Familiarity with healthcare processes and ability to collaborate with providers and patients to coordinate care
- Proficiency in using electronic medical record (EMR) systems and technology tools
- Understanding of functionally-informed and preventative healthcare approaches is a plus
- Ability to multitask, prioritize, and maintain attention to detail in a fast-paced healthcare setting
- Previous experience in a clinical or medical setting is highly desirable
- Healthcare certifications or relevant coursework in health administration or a related field are an advantage
