As a Certified Community Behavioral Health Clinic, CNS Healthcare's mission is to serve the people of our communities. The Assertive Community Treatment (ACT) Registered Nurse serves as a member of a community-based mental health team that provides services for individuals experiencing serious mental illness. The ACT Registered Nurse provides integrated behavioral and physical healthcare including medication management, the assessment and coordination of physical/medical care, and the provision of direct services to the beneficiary in the community. The mission of ACT is to help people become independent and integrate into the community as they experience recovery through a team-based service model.

Please note: As a community-based role, this position will render at least 70% of services in the beneficiary's home or other agreed-upon community-based position.

  • Provide integrated behavioral and physical healthcare, including medication management, the assessment and coordination of physical/medical care, and the provision of direct services to the beneficiary in the community.
  • Complete a thorough and accurate nursing assessment to evaluate current medical/physical symptoms and conditions, including substance use, and provide recommendations to applicable care providers as appropriate.
  • Contribute to the person-centered planning process for individuals served as a member of team-based service model and identify and use effective advocacy strategies to address and overcome diverse challenges facing individuals served.
  • Routinely collect accurate vital signs including, but not limited to, blood pressure, pulse, height, weight, body mass index (BMI), waist circumference, pulse, oxygen, glucose, pulse oximetry, and carbon monoxide testing.
  • Perform Point of Service testing including, but not limited to, HGBA1C, Drug Screening, Lipids, Glucose, and pregnancy testing as appropriate.
  • Assess and recommend lab work based on the individual’s medications and health conditions, collecting samples as required. Communicate with the individual, care team, and lab to ensure that lab work is successfully completed.
  • Coordinate, order, administer, and document injections to individuals served as requested by the prescriber. Ensure compliance with injection treatment by notifying the interdisciplinary care team when injections are missed.
  • Coordinate integrated care with internal and external providers of mental health, physical health, and substance use disorders including facilitating weekly prescriber appointments.
  • Act as a triage nurse to assist in responding to medical emergencies, triage emergent situations, and assist in problem-solving regarding medications, medical issues, or access to prescriber services as needed.
  • Provide education regarding chronic health conditions, disease process, healthy lifestyle management, vaccinations, and medication management including risks, benefits, and side effects.
  • Routinely review current medications with persons served and provide medication continuity until the individual is seen by an agency provider.
  • Recommend and encourage the use of increased prescriptions for Medication Assisted Treatment (MAT) for tobacco, alcohol, and opioid use disorders when clinically appropriate.
  • As a team-based service, perform a minimum of 80% of ACT contacts in the beneficiary’s home or other agreed upon community location, with at least 70% of the nursing contacts being performed in the beneficiary’s home or other agreed upon community location.
  • Provide transportation assistance to individuals served as needed.
  • Attend ACT team meetings at least weekly, or more frequently at the discretion of the ACT Team Supervisor, to review status of all beneficiaries, updates from on-call, clinical and case/care management needs, crisis management, schedule organization, and finalized plans for ACT staff deployment in the community.
  • Participate in on-call rotation as assigned, which includes completing pre-hospital admission assessments as needed.
  • Adhere to and maintain compliance proper guidelines and procedures to maintain a safe, hygienic environment including infection control, medical waste, pharmaceutical waste, and sharps disposal.
  • Assist in the management of medication supplies as needed.
  • Complete all required documentation completely, accurately, and in accordance with agency policy.
  • Meet direct service productivity and documentation standards per agency and service expectations.
  • Encourage the utilization of the patient portal system by creating a patient portal account for each individual served as needed.
  • Use motivational interviewing strategies to engage and provide service to individuals.
  • Utilize a trauma-informed approach to deliver services.
  • Attend and conduct meetings in-person in the office or the community, virtually through video (audiovisual), and by phone (voice only) as required.
  • Travel off-site as needed for various opportunities including meetings, conferences, activities, and events.
  • Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff.
  • Participate in and promote departmental and agency quality improvement initiatives.
  • Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
  • Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs.
  • Perform other related duties as assigned. 

 Qualifications:

Required

  • Associate degree in Nursing from an accredited school of nursing
  • Current, unrestricted, State of Michigan Registered Nurse (RN) licensure
  • Obtain and maintain Cardiopulmonary Resuscitation (CPR) and Basic Life Saving (BLS) certification within 60 days of hire
  • Valid and unrestricted Michigan Driver’s License
  • Reliable transportation and current proof of auto insurance
  • Prior clinical experience and demonstrated proficiency in providing clinical and nursing services including, but not limited to: collecting vitals, performing injections, and point of care testing
  • Prior clinical experience and demonstrated proficiency in medical management, crisis, and behavior management techniques
  • Working knowledge of medical terminology used in a mental health setting
  • Ability to share and teach on the continuum of drug and substance use
  • Must participate in MDHHS-approved, ACT-specific initial training within 6 months of hire, and subsequently participate in at least one MDHHS-approved, ACT-specific training annually thereafter.
  • Ability to work as a member of a multi-disciplinary team.
  • Experience with the application of the Health Insurance Portability and Accountability Act (HIPAA) in health care organizations
  • Proficient with computers and Microsoft office products
  • Proficient in the use of electronic health records (EHRs)
  • Ability to use discretion and judgment when handling matters of a sensitive or confidential nature
  • Advanced time management, problem solving, customer service, interpersonal, conflict resolution and crisis intervention skills
  • Ability to be forward thinking and take initiative to accomplish goals and objectives of the department
  • Ability to work in a fast-paced office environment with the usual pressures of managing multiple priorities with time constraints
  • Ability to communicate effectively, professionally, and courteously

Preferred

  • Bachelor of Science degree in Nursing from an accredited school of nursing
  • At least 1 year of experience as a registered nurse providing face-to-face, clinical services in any of the following settings: Behavioral Health, Community Mental Health, Psychiatric Hospital, State facility, Ambulatory Behavioral Health, Crisis Center, Substance Use Facility, Partial Hospital Program, Residential Program, or similar
  • Proficient with health registries including the Michigan Automated Prescription System (MAPS), PMP AWARxE Program, Prescription Drug Monitoring Program (PDMP).
  • General knowledge of the community mental health system based on the Michigan Mental Health Code and MDHHS contracts
