Colorado Palliative & Hospice Care Colorado Springs is looking for a full-time Clinical Director who leads by example and operates within the framework of the mission, standards, and philosophy of Hospice. The Clinical Director facilitates the interdisciplinary team process to deliver care and support to the patient, caregiver, and family.
Requirements
Qualifications:
- Current RN license in good standing to practice in respective state/states.
- 3 years of Hospice experience preferred.
- 1 year of management experience preferred.
- Certified in Hospice & Palliative Care Nurse preferred.
Role Expectations:
-May assume role and responsibilities as RN Case Manager as assigned depending on office census and Executive Director direction.
-Consistently practices and teaches Perfect Visit model to new and existing staff.
-Knows and follows the Conditions of Participation
-Be a knowledgeable resource to the interdisciplinary team regarding pain and symptom management, the Organization's standards, as well as Medicare guidelines and National Hospice and Palliative Care Association Standards of Care.
-Acts as a mentor to staff by scheduling, at a minimum, quarterly ride-a-longs, and one-on-one meets to assure processes (Perfect Visit) are being carried out.
-Works with Preceptor on new employee's clinical orientation and meets with new employee per schedule.
-Reviews the CTI brief narrative for completeness. Ensures that verbal certification orders are obtained from the Medical Director for each certification/recertification period. Ensures that a face-to-face encounter is completed and is available to the physician prior to completion of the brief narrative and signing of the document.
-Accountable to the admission, IDT, chart audit, QAPI, and death/discharge process.
-Reviews medical records of referrals to verify eligibility of hospice services.
-In collaboration with the Executive Director ensures that the agency remains survey ready.
-Collaborate with the Executive Director to ensure payroll is completed with accuracy and timeliness by ensuring all visits for the payroll period are documented and verified.
-Reviews on-call schedule to ensure adequate staffing weekends/holiday.
-Collaborate with Executive Director to manage to The Model including agency expenditures, pharmacy, staffing, etc.
-Collaborate with the Executive Director to ensure billing process is completed each billing cycle.
Salary : $100,000 - $110,000
