Under general supervision, works independently and as a member of the clinic based Care Management team to support care management services to high risk patients. Optimize patients’ progress through the continuum of care. Working in a collegial manner, helps case management develop thoughtful, effective, individualized discharge plans.
Job Requirements
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
High School Diploma/G.E.D. Required
2 years of experience in Behavioral Health or an associate's degree in Psychology, Social Work, or a related human services field. At least 1 year of experience working in an acute care setting such as an Emergency Department (ED) or as a Behavioral Health Navigator. Experience in the community
BLS Required within their first year of becoming a level II BHA, you must complete SBIRT (Screening, Brief Intervention, Referral to Treatment) training and the hours of observation required for billable services. Must also complete within 6 months the Behavioral Health Initiatives Training Framework that includes our package Behavioral Health curriculum and Allyship train-the-trainer training. The Specialist's area of specialty may include other train-the- trainer specific courses
