Responsible for the oversight and direction of the stroke program as defined by the Stroke Program Director and Joint Commission standards. Accountable for the planning, implementation, evaluation and revision of systems that support optimal stroke care and meet the goals of the stroke program. Works with the Stroke Program Medical Director to assure progressive and efficient stroke patient care from pre-injury to post-discharge related to Joint Commission standards and evidence-based practice for this patient population. The Stroke Program Coordinator provides leadership in the planning, development, and implementation of program goals and operations.

• Provides leadership for development of a comprehensive stroke program ensuring regulatory standards are met by conducting gap analysis, coordinating resources, facilitating inter-hospital collaboration, and working with the Transfer Center to facilitate patient transfers / admissions.
• Develops and implements clinical and organizational systems of facilitate meeting the needs of Stroke patients across the continuum of care by; developing and implementing policies and procedures and clinical management guidelines for the Stroke programs.
• Works collaboratively with department managers and directors to assess and revise departmental policies and procedures and clinical management guidelines to assure high quality care of stroke patients.
• Identifies and analyzes impediments to delivery and continuity of care while serving as an internal resource for staff in all departments and an extended liaison to EMS agencies, community hospitals, etc.
• Improves stroke patient care processes and outcomes by utilizing CQI principles as a basis for problem resolution and developing a quality improvement program reflective of the stroke patient population.
• Ensures staff and physician participation in quality improvement initiatives, by leading and facilitating interdisciplinary quality initiatives.
• Promotes research-based practice which incorporates scientific findings.
• Develops and implements a system for variance tracking by collecting and analyzing variance data and developing strategies for variance resolution.
• Ensures stroke program is in compliance with all requirements of regulatory agencies by maintaining certification requirements for the stroke center with the Joint Commission, leading site reviews when scheduled, and directing application process of Stroke Center with reviews/revisions as indicated
• Collaborates and maintains liaison role with Stroke Management related agencies; addresses system quality management issues as they arise; troubleshoots and problem solves regarding system issues; changing trends, legal regulatory standards, etc

MINIMUM

• Bachelors Degree in Nursing (BSN) or equivalent education and experience. 
• Minimum of 5 years nursing experience including care of Stroke Patients required
• Demonstrates good capacity for teaching, organizing, planning, and development.
• Strong interpersonal communication and customer relation skills.
• Working knowledge of performance improvements.
• Strong computer knowledge
• Registered Nursing License (RN) in state of practice
• American Heart Association Basic Life Support (BLS) 

PREFERRED

• 2 years experience in leadership type position or program development preferred.
• Specialty certification in neuroscience and/or critical care preferred
• Experience with spreadsheets, database and graphic applications.

Salary : $18,000
