Dietitian, Part-Time, Day Shift, Kenosha/Racine area Community Care is hiring a Dietician to join our team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This part-time opportunity offers a weekday schedule. It is based at our Vliet facility (currently telecommute is more the norm). Dietitian Responsibilities: ·
Implements the nutrition care plans for members.·
Promptly reports any unusual or sudden changes in a members’ condition or behavior.·
Responsible for the management of therapeutic diets, including modifications of the general menu to meet individual member needs and physicians’ orders.·
Documents nutrition notes in the member’s electronic medical record. Dietitian Requirements: Education: Bachelors degree in Dietetics Experience: A minimum total of one year of personal, clinical/internship, and/or employment experience providing care or services for adults with disabilities and elderly population required. Certification/Licensure: Registered Dietician through the American Dietetic Association or Certified through the State of Wisconsin. Valid WI Driver License Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage. Dietitian Job Schedule:·
Monday to Friday, 20 hours a week·
