Job Title: Case Manager
Reports to: Case Management Supervisor
Primary Job Function: Under minimal supervision, provides case management along with service and resource coordination in a human services environment.
Duties and Responsibilities:
· Conducts phone intake assessment and in-person appointments. Manages documentation for organizational purposes, as well as management and reporting for state agencies
· Counsel’s clients and/or their families to facilitate achieving service plan goals, developing life skills, and or sustaining financial stability, conducts activities and monitors the service environment in a manner to maximize client success and well-being
· Attends community meetings that pertain to resources specific to waiver services
· Works with community partners to maximize the assistance for clients
· Advocates on behalf of clients and families for services, basic needs, and other related issues
· Composes or prepares correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications
· Conduct comprehensive, client-centered social work activities in accordance with best practices
· Stay abreast of community resources and procedures for referring clients to appropriate resources
· Performs other duties and assumes other responsibilities as assigned by the supervisor
Education Requirements
· Bachelor’s degree with major course work in a health or human services field from an accredited college or university
· At least two (2) years of assessment and care planning experience with clients (experience cannot include more than six (6) months of internship)
· Knowledge of general interviewing practices and/or techniques; knowledge of social work and counseling practices and techniques, including crisis prevention and intervention

Position Requirements
· Must be available to attend and successfully complete agency contract training requirements
· Knowledge of Microsoft Office Suite, Google Workspace, and ability to work in online platforms
· Must be available Monday through Friday 8:00am-5:00pm, excluding holidays and weekends
Environment and Physical Demands
Work is primarily performed in an office setting that is climate controlled (i.e., heated, and air-conditioned). Lighting is adequate through overhead lighting, as well as windows and blinds. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Physical demands in both work environments include sitting and/or standing for extended periods of time, bending, lifting approximately 25 pounds, regular use of the telephone, copier, fax, and computer. Demands also involve protracted concentration.

Work Hours
Normal business hours, Monday through Friday from 8:00am-5:00pm.
Job Types: Full-time, Part-time
Pay: $37,787.00 - $38,905.00 per year
Expected hours: 15 – 40 per week
