Job Summary :
This position is a full-time position consisting of 40 hours per week. CORE Peer Specialist will promote mental and physical wellness through evidence-based practices and personal experience sharing. Position is based in Malone NY to serve the Franklin County NY communities.
Qualifications :
High School Diploma or GED required.
NYS Peer Specialist Certification required.
Reliable and trustworthy, sound decision-making skills.
Strong verbal, writing and organizational skills.
Strong computer skills (Microsoft Word, Excel).
Demonstrated ability to serve individuals and assist interagency and outside agency employees in a professional, welcoming, and efficient manner.
Cooperative, friendly, and helpful attitude with individuals and co-workers. Ability to work closely with other employees to ensure a positive, constructive environment within individual programs and overall agency.
Ability to work independently and in a team setting.
Ability to maintain confidentiality regarding the agency’s functions and individuals receiving services.
Reliable transportation including a valid driver’s license, registration, inspection, and insurance.
Must be willing to self-disclose personal recovery.
Knowledge of trauma and its effect on an individual.
Must positively promote the agency within the community.
Must be detail oriented.
Must be able to clear NYS Justice background check, NYS Office of Children and Family Services background check and clear the NYS staff exclusion list.
Must be willing to travel 75% or more per week.
Job Responsibilities :
Must complete Mandated Reporting and HIPAA training within the first week of employment.
Must complete mandatory trainings through (CPI) Center for Practice Innovations
Must obtain an additional 10 hours of CEUs annually after receipt of New York State Peer Specialist Certification to maintain credentials
Keep statistical data to report to direct supervisor by the last working day of the month.
Must have knowledge of resources and systematic processes in Clinton County.
Must continually learn system processes for all systems in Clinton County such as, but not limited to, courts, probation, Department of Social Services, Public Defenders, Conflict Defenders.
Must abide by all agency policies and procedures.
Must maintain the confidentiality of individuals worked with and agency functions.
Responsible for assisting with daily upkeep and cleaning of the agency, including copier and fax machine.
Engage in supervision meetings with direct supervisor.
Help to connect individuals to providers they deem suitable for their overall well-being such as, but not limited to, primary care, etc.
Ability to focus on the root cause of issues and not just the issue presented.
Complete continued education on recovery to help individuals you are working with.
Maintain a professional working relationship with all agencies on behalf of CCFC.
Must always deliver a person-centered service model.
Review social determinants that may be affecting an individual or family’s overall health.
Complete daily detailed person-centered progress notes for individuals you are working with the same day of services with the individual.
Maintain outcomes for recovery as part of progress notes.
Adequately maintain files per CCFC guidelines.
Promote community integration and involvement.
Empower individuals you are working with by offering information and education.
Refer individuals to services they need within Clinton County.
Work with and educate all systems on CCFC Services within Clinton County in a professional manner.
Educate individuals and families about service providers they may be involved with such as state and federal requirements of agencies.
Any other job duty as assigned by direct supervisor.
