Community Health Alliance is looking for full-time Population Health Medical Assistant. Join our talented team of Population Health Medical Assistant that focus on serving the community and helping those in need by creating healthy outcomes for patients of every income.
We operate seven convenient locations throughout Reno and Sparks, providing comprehensive, top-quality medical services, dental and pediatric care, behavioral health, low-cost pharmacies, nourishing food pantries, and more.
The Population Health Medical Assistant will support Community Health Alliance's ongoing clinic operations and Population Health initiatives. This position is competent in managing and performing outreach efforts, patient care forms, and patient chart review. This position will have the ability to extract data reports which support population health, contractual obligations, and efforts to address prioritized HEDIS metrics. This position is responsible for managing and scheduling hospital discharge follow-up appointments and other special project outreach efforts. This position conducts the coordination of care to improve gap closures rates such as hemoglobin A1c, preventative screenings, and medication adherence. This position is a licensed Community Health Worker addressing the medication adherence of Community Health Alliance patients. This position is responsible for completing chart scrubbing/appointment preparation that aligns with the Population Health team goals and project outcomes. This position works with and supports health plan management, medical leadership, clinical operations, and contract compliance. All work is done to ensure compliance with internal policies, external regulatory agencies, and industry best practices.

Qualifications
Education and Experience: Applicants must have at least two years of medical assistance
experience. Medical Assistance certification and experience as a medical assistant with a
Federally Qualified Health Center (FQHC) preferred but not required. Must be licensed
Community Health Worker in the State of Nevada or become fully licensed within one year of
hire. Experience working with an Electronic Health Record (EHR) System required. Meaningful
experience (2 years) with HEDIS, NCQA, UDS, and gap closure analysis preferred.
Bilingual Spanish required.
