Clinical Admin

Description

 The Clinical Admin will provide administrative support to primary care teams in facilitating patient care and promoting positive health outcomes.

.  Duties will include:

• Patient Care Coordination in triaging calls and My Chart Messages, routing to appropriate team members and closing communication loop with patients as needed. 

• Supporting Quality Measures, ensuring patients are current with all Health Maintenace Goals and follow up visits, ensuring access to care.

• Bridging barriers to care, including arranging for transportation, translation services, coordination with outside agencies and completing paperwork required.

• Documenting and updating computer systems (EPIC, Sorian) to order and coordinate patient services

Qualifications

• Candidates will need a working knowledge of medical terminology and have 2 years supporting a patient care team.
• Strong organizational and computer skills are required, with demonstrated ability to thrive in a collaborative, fast -paced environment!
