Integrated Care Coordinator
We are seeking a full-time Integrated Care Coordinator to provide integrated care management services across all services lines (medical, dental and behavioral health). Care management services include but are not limited to: screening, brief intervention, case management, education and coordination within clinics and community resources. This position will rotate between the CHC’s three sites.
Successful candidates will be skilled at making quick and accurate clinical assessments of mental health and substance use disorder conditions. Candidates should have knowledge of the principles and practices of behavioral health consultation, including assessment, brief intervention, and motivational interviewing skills and their application to population-based care for preventative and chronic care issues.
Must possess a Bachelor’s degree in psychology or social work; Master’s degree preferred. The ideal candidate would be licensed-eligible or maintain an independent license to practice counseling or social work.
