Position Title: Nurse II: Certified Community Behavioral Health Clinic (CCBHC) & Community Action Team (CAT)

Location: Martin Luther King Jr. Clinica Campesina, Everglades Health Center and Community- Based

Job Type: Full-Time, Non-Exempt

Position Purpose: The Nurse II delivers comprehensive nursing care to clients enrolled in the Certified Community Behavioral Health Clinic (CCBHC) and Community Action Team (CAT) programs. This is a dual-role position and collaborates with an interdisciplinary team to ensure the highest quality of patient-centered care, supporting clients with mental health, substance use, and primary care needs.

Position Requirements/ Qualifications:

Education and Experience:

Graduate of an accredited Associate Degree in Nursing or a Bachelor of Science in Nursing (BSN) program. Experience in behavioral health, community health, or working with individuals with co-occurring mental health and substance use conditions is preferred. Active Registered Nurse (RN) license in the state of Florida.


Licensure/Certification:

Licensed to practice in the State of Florida. Maintain current BCLS certification from the American Heart Association. Maintain current First Aid certification; First Aid certification must follow American Heart Association guidelines. I.V. certified desired.


Skills and Abilities:

Strong clinical assessment and problem-solving abilities. Effective communication and interpersonal skills to collaborate with a multidisciplinary team. Proficiency in health assessment, screenings tools, has behavioral health knowledge and ability to document in electronic health records. Bilingual candidates are preferred to better serve diverse patient populations.


Position Responsibilities:

· Provide direct nursing care to patients in clinic, home-based, and community settings.

· Conduct clinical assessments, monitor health status, and deliver timely interventions.

· Collaborate with primary care physicians, psychiatric providers, social workers, and other team members to develop individualized treatment plans.

· Administer medications and treatments as prescribed, ensuring safe and effective implementation. Conduct drug screenings as needed.


· Health Education and Preventive Care, educating patients and families on mental health, substance use prevention, chronic disease management, and lifestyle changes.

· Provide psychoeducation on depression, anxiety, trauma, and other behavioral health conditions.

· Conduct health screenings for diabetes, hypertension, and obesity, offering preventive recommendations.

· Coordinate referrals to external providers as needed and collaborate with community partners and resources to support comprehensive patient care.

· Ensure continuity of care through follow-up visits and monitoring progress.

· Complete accurate and timely documentation in electronic health records within the same day of intervention.

· Ensure compliance with organizational policies, procedures, and standards of care, including infection control and safety protocols.

· Participate in interdisciplinary team meetings and performance improvement activities.

· Maintain effective communication with team members to ensure seamless coordination of patient care.

· Support clients through behavior modification, psychoeducation, and referrals for smoking cessation and substance use treatment programs.
