What are the responsibilities and job description for the OUTREACH WORKER- HEALTHY STEPS position at COMMUNITY HEALTH OF SOUTH FLORIDA INC?
Position Purpose: The Outreach Worker will be responsible for collecting assessments from children and their families ages 0-3 that are seen in Family Medicine. The Outreach Worker will also be responsible for mapping of community resources in order to meet the needs of consumers as well as linking consumers to services, mentoring consumers, and bringing awareness to the community about the program.
Position Requirements / Qualifications:
Education/Experience:
High School Graduate or GED Equivalent.
Previous experience with outreach desired.
Licensure / Certification:
Maintain current CPR certification from the American Heart Association. Valid Florida Drivers License.
Skills / Ability:
Good communication and interpersonal skills. Sensitivity in working with diverse cultural groups bilingual preferred.
POSITION RESPONSIBLITIES (THIS IS A EXEMPT POSITION)
- Promote the HealthySteps program to families being seen in Family Medicine
- Assist in marketing and advertising for the program.
- Assist with identifying and removing barriers through community resource mapping for patients in the areas of child development
- Collaborate with existing programs and departments to ensure program awareness.
- Act as a liaison between HealthySteps at CHI and the community.
- Maintains accurate records of the activities performed and posts all activity daily.
- Disseminates the program literature.
- Administer assessment to all children and their families ages 0-3 in Doris Ison Family Medicine
- Assist with follow-up of consumers as needed.
- Attends scheduled CHI in-services and annual updates.
- Maintains regular contact with consumers to ensure their needs are met and assist in guiding them services.
- Follows CHI Policies and Procedures.
- Ensure all documentation is completed within 24 hours of service as well as all billing and encounters.
- Adhere to scope of service of funders.
- Maintain personal database of all community contacts and resources on a regular basis accurately.
- Report all incidents to supervisor immediately.
- Adhere to AIDET requirements.
- Meets monthly goals and submits monthly reports timely.
- Reports to work on time with minimal absenteeism.
- Performs other duties as assigned.
