Description

Heath Information Management (HIM) Specialist
Department: Quality Improvement
Time Commitment: Full-time
Reports To: Health Information Management Coordinator
FLSA Status: Non-Exempt
Primary location: West Liberty, Ohio (All locations)

The HIM Specialist will process, transfer, manage and ensure security of patient data that come into and exit the health center. Assist with data collection and quality improvement initiatives and work to improve the practice and care for patients by accurately distributing and managing electronic and hardcopy patient documents and information. Manage patient release of information concerning HIPAA and any other regulations.

Primary Accountabilities

•Manages/assists with all patient documentation and administrative processes as assigned
•Ensures all information, scanning is accurate, current, and completed in a timely manner.
•Ensures all patients served experience a high level of customer satisfaction and care
Operational Excellence
•Ensures that all documentation that is collected, processed, entered, and filed is done timely and accurately
•Ensure and uphold the confidentiality requirements of all patient records and manage all daily tasks and activities consistent with HIPAA, state and federal laws and regulations, as well as the health center’s policies and procedures regarding confidentiality and security.
Relationship Management
•Develop and ensure favorable relationships with all patients and their families.
•Develop and ensure ongoing, positive relationships and collaboration with providers, nurses, administrative and clinic operations staff.
•Ensure the success of each clinic operation through collaborative support and working relationships with all clinic operations staff.
Professionalism & Stewardship
•Ensure all actions, job performance, personal conduct and communications represent the
organization in a highly professional manner at all times.
•Uphold and ensure compliance and attention to all corporate policies and procedures as well as
the overall mission and values of the organization.
Health Information Management Specialist
Primary Tasks and Duties
• Manage incoming and outgoing patient healthcare documents, i.e., sorting faxes, following provider attention to fax, attach to patient chart and fax back to originator, if necessary
• Send appropriate labs to patient portal
• Route secure messages to appropriate recipient
• Address patient record release requests, route and follow with provider
• Address skilled care pharmacy requests, route and follow with provider
• Address signed nursing home facility notes to appropriate nursing home facility
• Attached scanned documents to patient’s chart
• Address Pre-op requests; send reminder to nursing on day of appointment that pre-op request is
on chart. Also send reminder to lab
• Diabetes Tracking: collect diabetic information from patient chart for all diabetic patients.
Transfer information
• Transitions of Care module Management and document retrieval
• Other duties and responsibilities as assigned

Essential Duties and Key Competencies
•Strong attention to detail and accuracy.
•Ability to utilize computers for data entry and information retrieval.
•Excellent verbal and written communication skills.
•Ability to implement and evaluate operational and administrative processes.
•Participate in aspects related quality compliance, improvement, and risk management for a Federally Qualified Health Center
•Engage in certification projects, i.e. PCMH, TJC
•Engage in all corporate compliance requirements of Community Health & Wellness Partners

Work Environment
The work environment characteristics are those for a normal provider office environment. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work may require occasional weekend and/or evening hours.
Work hours
Full-time, non-exempt hourly position. Generally, a 40-hour work week is required for full-time; part-time as agreed upon by parties; hours are subject to change. Office hours vary by site but are usually as early at 6:30am to as late as 7:00 pm, Monday through Friday. Some Saturdays and after business hours work may be required. Assigned meetings and travel are a part of the position.
Wages
Starting wages for Health Information Management Specialist will be negotiable based on Community Health & Wellness Partners pay policies and factors such as education and experience, national and state average compensation recommendations based on location and region.
Introductory Period
90-day probationary period. Probationary period may be waived or reduced if employee is transferred into the role through lateral position change or promotion.

Qualifications


Qualifications and Requirements
Education

•Associate degree or 3-5 years’ related experience required.
•Knowledge of EHR systems and practice management. ECW experience Preferred.
•Experience with HIPAA preferred
•Knowledge of IT troubleshooting
Certificates and Licenses
•Valid Ohio driver's license

Physical/Environmental
•Occasional lifting of 40 – 50 lbs. and pushing of 5-20 lbs.
•Sitting, standing, and walking approximately 80% of the time

•Bending and reaching approximately 20% of the time
•Normal accessibility and mobility throughout the region required
•Normal overtime/extended work hours
•Travel is required between all CHWP locations as needed.
