Coverage Area: Fountain, Montgomery & Southwest Tippecanoe Counties Schedule: Full-Time, Mon through Fri About the Role As a Home Health Registered Nurse, you’ll deliver skilled nursing care to patients with acute, chronic, and/or terminal conditions who meet home health admission criteria. You’ll provide care in patients’ homes or facility settings, manage a caseload, complete admissions and assessments, and deliver ongoing skilled visits in compliance with regulatory requirements. You’ll serve as a key coordinator of care, collaborating closely with physicians, the interdisciplinary team, patients, families, and caregivers to ensure each patient’s plan of care is comprehensive, effective, and compassionate. Autonomy and flexible scheduling Meaningful 1:1 patient care Supportive, collaborative team environment Competitive compensation Tuition reimbursement for full-time employees and continuing education opportunities for all staff Comprehensive medical, dental, and vision benefits 401(k) with employer match Paid time off, paid holidays, and family & pet bereavement Pet insurance What You’ll Do Deliver skilled nursing care aligned with Elara Caring’s mission, values, and clinical standards Perform admissions, initial assessments, and ongoing reassessments during each visit Maintain primary responsibility for assigned caseload, including planning, implementing, coordinating, and evaluating patient care Identify and address patient, family, and caregiver needs—physical, psychosocial, environmental, and spiritual Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team and attending physician Communicate significant changes in patient condition or environment to physicians, Clinical Supervisors, and care team members Ensure accurate, timely documentation in accordance with regulatory, payer, and organizational requirements Maintain patient and staff privacy and confidentiality in compliance with HIPAA What We’re Looking For Current, unrestricted RN license in the state Minimum of 1 year of nursing experience in a clinical setting Ability and willingness to travel within the branch coverage area (approximately 50%) Valid driver’s license, dependable vehicle, and current auto insurance Associate or Bachelor’s Degree in Nursing (preferred) Previous Home Health or Hospice experience (preferred) Experience with HomeCare HomeBase (preferred) If you’re ready to bring your compassion, clinical expertise, and independence to a role where you truly make a difference every day—apply today and become part of the Elara Caring family.
