Coverage Area: Fountain, Montgomery & Southwest Tippecanoe Counties Schedule: Full-Time, Mon through Fri About the Role As a Home Health Registered Nurse, you’ll deliver skilled nursing care to patients with acute, chronic, and/or terminal conditions who meet home health admission criteria. You’ll provide care in patients’ homes or facility settings, manage a caseload, complete admissions and assessments, and deliver ongoing skilled visits in compliance with regulatory requirements. You’ll serve as a key coordinator of care, collaborating closely with physicians, the interdisciplinary team, patients, families, and caregivers to ensure each patient’s plan of care is comprehensive, effective, and compassionate.
