Job Description
As a Home Care Coordinator (HCC) with Community LIFE, you will be instrumental in assessing participant needs and coordinating the delivery of personalized home/personal care services.
Your primary focus will be on maximizing rehab potential in daily living and minimizing or eliminating inpatient care.
You will work under the supervision of the Home Care Supervisor and collaborate with the Therapy team to develop an appropriate plan of care for in-home services.
Additionally, you will participate in weekend and on-call rotations.
Required Skills and Qualifications
- Knowledge of the medical, social, and emotional needs of frail elderly populations.
- Effective written and oral communication skills.
- Strong organizational and planning abilities, with the capacity to manage multiple priorities.
- Critical decision-making skills, allowing you to work independently.
- Working knowledge of utilization review, quality assurance, and managed health care concepts.
- Ability to work collaboratively within an interdisciplinary team approach to care for the elderly.
- Basic working knowledge of Windows operating systems, email, and word processing.
- Able to deliver services in a compassionate, responsive, and courteous manner.
- Dependable, resourceful, and flexible.
- Ability to work effectively with staff, participants, providers, and referral sources.
- Interest in geriatrics and community-based programming.
- Appreciation for working with elderly individuals.
