What are the responsibilities and job description for the Service Coordinator/Case Manager position at Community Living Case Management?
We are seeking qualified applicants for a Service Coordinator/Case Manager position to work with adults and/or children with intellectual and developmental disabilities. This position is responsible to facilitate services for a caseload of adults and/or children with I/DD, meet with families, assess support needs, and develop service plans for individuals supported.
This position will require routine communication with individuals, partner agencies, families, and caregivers.
Required Qualifications:
- A bachelor's degree in human science, social science, or behavioral science; OR
- A combination oeducation and human services/social services experience.
- Good verbal and written skills
- Ability to work with individuals and families under stressful situations
Preference given to bilingual candidates
Job Type: Full-time
Pay: From $24.00 per hour
