JOB DESCRIPTION


TITLE:                                    Quality Improvement and Compliance Coordinator

BASIC FUNCTION:               This position is responsible for managing the quality improvement program and compliance as it is related to federal, state and county contracts, service utilization and outcome measures.

SCOPE:                                             Supervisor: Executive Director of Community Affairs
                                                Supervisee: None


ESSENTIAL FUNCTIONS:

    1. Oversee and coordinate all compliance initiatives to ensure that CLASS remains in the highest standing with all regulatory entities.
      1. Coordinate all external program audits which include but are not limited to 2380 licensing, 6400 licensing, QMET, AAA Options, CARF, Department of Health, QA&I, MCO’s, and various private contracts.
      2. Review individual’s records at least twice a year to assess/ensure compliance with internal and external policies, procedures and standards.
      3. Maintain, update, and/or write agency policy and procedures related to funder or accreditation oversight.
      4. Communicate with regulatory agencies and insurance companies.
      5. Monitors best practices regarding service provision and assists in ensuring that services meet federal, state or local government quality standards.
      6. Complete annual self-assessments for program services in preparation for monitoring.
      7. Ensure Fiscal Department completes regular audits regarding funders’ billing requirements as required by CARF.
 
    1. Facilitate all quality improvement activities to ensure that services are delivered at the highest standards.
      1. Maintain data for key performance indicators for all programs.
      2. Assists supervisory staff and teams in developing, implementing and tracking quality improvements indicators.
      3. Collect and analyze output and outcome data for each program.
      4. Facilitate quarterly quality meetings with staff to review quality metrics.
      5. Write, implement, and monitor plans of correction for all agency monitoring.
      6. Monitor authorizations from ODP for all programs and from MCO’s for residential services.
      7. Conduct consumer/family satisfaction surveys annually and analyze results to be presented in the quarterly quality meeting.  
 
    1. Ensure compliance with all incident management activities and facilitate efforts to reduce the occurrences of critical incidents.
      1. Receive notices of all potential incidents and determine if they are reportable.
      2. Ensure that all identified incidents are entered into the EIM system by staff in a timely manner and, at times, enter them directly.
      3. Ensure that all required investigations are completed in a timely manner and finalized in the EIM system.
      4. Assist with any corrective actions which could include staff trainings, updating policies/procedures, writing policies/procedures, among others.
 
    1. Improve staff knowledge and retention through identifying and providing trainings to ensure a high level of quality and compliance.
      1. Ensure that compliance issues and/or information is addressed in staff meetings of all programs through direct involvement or through information supplied to managers.
      2. Identify gaps in services and recommend training topics to directors and managers.
      3. Directly provide and/or coordinate trainings on quality and compliance topics as needed.
      4. Effectively work with all staff including leadership and the training coordinator to ensure staff are receiving appropriate training to improve the quality of care.
      5. Collaborates/works with others on the principles of quality and the value of meeting/exceeding quality standards.
      6. Provide updates to staff on all regulatory changes that are posted by the Department of Human Services, the PA Department of Health, CARF, or any other programmatic oversight entity.
 
    1. Build and maintain an organizational “culture that measures” by serving as the point person for the Therap database system.
      1. Work with program leadership to develop forms in the system that reflects the documentation needs.
      2. Pull data from the system through preset reports or created reports.
      3. Serve as a point person to assist staff with the usage of the system.
      4. Communicate with Therap staff as needed to resolve any database issues.
 
    1. Assist with improving and maintaining the health and safety of individuals served.
      1. Conduct monthly site safety inspections of residential and facility locations.
      2. Make recommendations to program directors and the facility director of any unsafe situations.
      3. Monitor personal protective equipment usage and order new supplies to maintain inventory.
 
    1. Other duties as assigned.

MINIMUM QUALIFICATIONS
  1. Master's degree in social work, education, or psychology with emphasis in human services plus one year supervisory experience in human services program development and implementation.  OR
  2. Bachelor's degree in same areas plus five years experience in human services program development and implementation. OR
  3. Equivalent combination of education and experience.
  4. Experience in Microsoft Office, particularly Word and Excel.
  5. Must have excellent writing and verbal communication skills.
  6. Working knowledge of applicable regulations and requirements.
  7. Current criminal history clearance.

Physical Demands/Requirements:
  1. Ability to see and interact with text and graphical information on a screen.
  2. Ability to receive and respond to phone calls.
  3. Ability to use office equipment.

Description of Work Environment/Conditions
  1. Periodically travel locally.
  2. Most work occurs Monday through Friday during business hours but some evening and weekend hours may occur. 
  3. Regularly work in a climate-controlled home or office environment or another approved location in the community.
