Per-Visit Admission RN
Requirements:
- Licensed RN in Massachusetts
- Graduate of an approve school for professional nursing
- A minimum of one year of recent experience in direct patient care nursing.
- Previous home care experience preferred.
- Experience with documentation in a patient Electronic Medical Record is a plus.
Community Nurse is looking for a full-time Admissions Nurse to join the Home Care Nursing Team. This position has a weekend and holiday rotation.
Skills Requirements:
- Able to assess and respond to the needs of patients and families in varied settings.
- Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
Responsibilities include but not limited to:
- Give skilled comprehensive nursing care to all age, gender, and cultural groups.
- Assess client status upon initial home visit to identify needs in support of development of comprehensive case management plan.
- Document in an accurate and timely manner not later than end of working day.
- Using basic teaching principles, counsel and instruct the patient, family and/or significant other in meeting nursing and related needs.
- Maintain regular communication with the Clinical Manager, and/or other team members concerning patient/family needs.
- Participate in Community Nurse's orientation and in-service training programs for professional staff.
- Participate in agency and community programs as requested to promote professional growth and understanding of home care.
- Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.
- Accept other assignments as appropriate.
Pay: From $50.00 per hour
