What are the responsibilities and job description for the DSP - Community Hab position at COMMUNITY SERVICES FOR EVERY1 INC?
Community Habilitation is a service that provides one-to-one support to people who live in the community with intellectual/developmental disabilities. Supports are provided to people within their homes and within their communities to develop or enhance the skills needed to live as independently as possible. DSPs in Community Hab empower the people they work with by modeling, teaching, and advocating for them to achieve their highest level of independent living, enable their fullest participation in society, ensure their civil and human rights are protected and promote their ability to be productive, contributing members of their community. Paid training is provided. We have full time, part time or per diem positions. Shifts are days, afternoons and/or evenings; locations are throughout WNY.
Responsibilities include:
- Providing transportation when necessary to people served to social events, appointments, and outings in their own personal vehicles. If using staff vehicle mileage reimbursement is provided.
- Provide support by demonstrating respect and teaching daily problem-solving skills.
- Providing support to develop and maintain positive interpersonal relationships with their peers, with their families and within their community.
- Completing service documentation in an electronic medical record (EMR) system in accordance with all applicable state and federal rules and regulations.
Education and Experience:
- H.S. Diploma/GED OR the successful completion of our internal assessment of reading/writing abilities.
- 6 months experience working with adults with developmental or intellectual disabilities.
- Valid NYS Driver License with acceptable driving record.
Skills, Knowledge, and Abilities:
- Use of a personal vehicle for work purposes which may include traveling between work locations and transportation of people served.
- Proof of valid insurance on the car you will be using for work purposes.
- Positive attitude with motivation to provide interactive and compassionate experience to those in our programs.
- Must be able to read and write case notes and be able to follow written directions.
- Must be able to manage multiple assigned tasks at one time and meet assigned deadlines.
- Must have basic computer knowledge.
Certifications:
- Within 120 days of employment, must become CPR, First Aid and CORE SCIP-R certified.
Physical and Mental Requirements:
- Must be able to lift up to fifty (50) pounds.
- Seeing, hearing, walking, sitting, standing, driving, stooping, bending, carrying, stretching, and climbing stairs as well as all movements required by SCIP-R curriculum.
