Job Title: Hospice RN
Job Type: Per-Diem or Part-Time
Location: Somerset & Warren County | Somerville, NJ
Per Diem Rates:
- Admission = $110.00 per visit
- Revisit = $70.00 per visit
- Recertification = $80.00 per visit
PART TIME: Starting From: $41.00/hr

Education/Experience:
- Current NJ RN License
- Valid Driver’s License required
- One year exp. in home health or hospice required
Job Description:
- Performs comprehensive assessments at initiation of services and ongoing.
- Develops, monitors and updates patient care plan with input from patient/family, attending Physician Hospice Medical Director and IDT Team.
- Communication with attending physicians, hospice team members and other community agencies.
- Maintains accurate and concise clinical records.
- Develops plan of care for home health aide.
- Documents all pertinent patient related information in the clinical record and reports significant changes to appropriate sources.
- Provides bereavement follow-up, as needed, in accordance with the bereavement plan of care and Bereavement policy.
- Participates as an active member of the hospice IDT, attending both patient care conferences and staff support meetings, as scheduled.
