Community Care Weight Management & Metabolic Care (CCPWMMC) is seeking a motivated Nurse Practitioner to join the team. CCPWMMC provides care to patients with conditions associated with metabolic syndrome, including obesity, hypertension, high cholesterol, and diabetes in addition CCPWMMC offers a comprehensive, multidisciplinary program to assist patients with weight loss.Responsibilities
- Assess patient status by obtaining health history.
- Order / perform laboratory diagnostic and other screening tests.
- Monitor therapies and provide continuity of care.
- Counsel patient / family on preventive health care.
