Your Responsibilities:

• As the MDS Lead, you’ll play a crucial role in coordinating and overseeing the assessment and care planning process. You’ll ensure compliance with federal regulations, optimize reimbursement, and collaborate across departments to provide high-quality resident outcomes.
• Manage the RAI process, including timely and accurate completion of all MDS assessments
• Lead interdisciplinary care plan meetings
• Educate and support clinical team members in MDS procedures and documentation
• Monitor quality measures and support improvement initiatives

What We’re Looking For:

MDS Experience Is a MUST – Minimum 2 Years Preferred

SNF experience.

Active RN license in PA

Strong knowledge of Medicare/Medicaid reimbursement and RAI guidelines

Detail-oriented, organized, and confident in leading a clinical team
