Role and Responsibilities
- Assists clients served by the Crisis Team in gaining needed access to medical, social, educational, financial, and other community support services within and outside the agency.
- Serve as a member of the multidisciplinary Crisis Team.
- Participate in daily team staffing and treatment plan review meetings.
- Provide follow-up of clients served by Crisis Team Attend staff meetings, in-service trainings and workshops as required.
- Advocates for, coordinates, and links clients with needed services and resources.
- Identifies barriers for accessing services and a plan to address them and ensure outpatient follow up care.
- Complete all documentation in a timely and accurate manner in accordance with medical records guidelines.
- Capable of visiting consumers in their own home which may not be handicapped accessible.
- Other duties may be assigned.
Qualifications and Education Requirements
- Bachelor of Arts or Science Degree in any of the behavioral sciences from accredited institution.
- Behavioral Sciences include psychology, social work, sociology, counseling and human services.
- One year of experience in performing case management services or working with behavioral health population.
- Master’s degree in behavioral science may substitute for the one year of experience.
Special Requirements
- Must possess a valid driver’s license.
Salary : $30,600 - $38,700
