JOB SUMMARY:
The Integrated Behavioral Health Clinician is embedded within the primary care teams and works directly with the teams to assess individuals with mental health and/or substance use challenges and provides brief treatment services within the primary care setting. The Integrated Behavioral Health Clinician’s goals are to help improve recognition, treatment, and management of behavioral health and medical conditions in the population. The Integrated Behavioral Health Clinician applies the biopsychosocial model to the primary care setting and demonstrates a basic knowledge of pharmacotherapy. The Integrated Behavioral Health Clinician works as a consultant to the primary care teams.
Requirements
REQUIRED QUALIFICATIONS:
- Must hold a master’s degree in Social Work or related field from an accredited college
- Licensed Clinical or Independent Clinical Social Worker in the State of Rhode Island
- Previous experience or internship in a medical setting which supports a primary care integration model preferred.
Work Schedule Demands:
- Full-time, part-time, and fee-for-service positions unless specified differently by the director
- Up to two evenings may be required
- May be required to work at any CCAP component and may be required to work in the community
- Due to the nature of the work and level of responsibility, this individual maintains a flexible schedule, which may go beyond a regular workweek
When the Integrated Behavioral Health Clinician is not in the primary care setting, recommended to be readily available by phone during usual duty hours. Although the Integrated Behavioral Health Clinician may not always be able to respond in person due to other duties, phone consultation, at a minimum, will be readily provided.
CONFIDENTIALITY OF INFORMATION:
Has unrestricted access to clinical records assigned to caseload. Must follow State and Federal Laws of confidentiality of Clinical Records and must follow CCAP policies and procedures for the creation, protection, retention, release, and disposal of Clinical Records.
KEY RESPONSIBILITIES:
Clinical
1. Consults with primary care providers to assist patients with psychosocial issues, frequently done as a “warm hand-off” during hours of clinical operation. Informal referrals from the primary care providers happen at other times as well.
2. Conducts intakes for CCAP’s Behavioral Health Services for “warm hand offs,” or make external referrals as necessary
3. Most patients are seen for three or fewer follow up appointments, when necessary, as a bridge to longer term treatment
4. Participates in team “huddles” within the primary care setting and utilizes behavioral health screens, as necessary.
5. Performs assessment and rapid problem identification
6. Demonstrates competence in and provides brief assessment, crisis management, and triage
7. Provides case management services as necessary.
8. Provides timely feedback about the patient’s care to primary care providers regarding consultation findings and recommendations.
9. Develops and maintains professional relationships with internal and external community health providers, schools, legal, welfare, and other social/human service and mental health personnel and agencies.
10. Conducts workshops, trainings, and educational programs to other CCAP components and the community specific to the Model of Integrated Care.
11. Utilizes the electronic health record to document all patient encounters.
12. Any other duties as assigned
Administrative
1. Completes and reports all documentation work in a timely manner.
2. Participates in weekly supervision (e.g., individual and/or group).
3. Manages the clinical record in accordance with CCAP policies and procedures.
4. Participates in performance improvement committees and/or other committees.
5. Participates in community boards, groups, and councils, as required.
6. Participates and assists in departmental studies or projects, as required.
7. Participates in professional responsibilities and activities including program development.
8. Participates in trainings required by CCAP and other trainings necessary for the maintenance of professional licensure and/or supervisory requests for growth and improvement.
9. Any other duties as assigned

The environment is a clinical business office environment. Potential exposure to blood bore pathogens, respiratory diseases and other infectious diseases, and other biohazardous material requiring universal standard precautions. Transportation is necessary at times. Must follow all CCAP Environment of Care and Infection Control Policies.
