What are we looking for in a Crisis Case Manager?
Crisis Case Managers work closely with our Crisis Outreach Professionals to support clients who are experiencing a behavioral health crisis. Crisis Case Managers offer supportive counseling to help clients stabilize and then assist clients with connecting to appropriate community services and treatment. Activities generally include gathering initial referral information, meeting with individuals to gather additional information on the presenting issue, reviewing this information with a Crisis Outreach Professional when necessary, and facilitating in-person or telehealth crisis evaluations. In addition to providing face-to-face client care, Crisis Case Managers also act as liaison between Comprehensive Healthcare and other community providers to coordinate services and ensure clients can access the resources they need. To be successful in this role, Case Managers must enjoy variety in their day, possess exceptional communication skills, and be able to quickly build rapport with people of all backgrounds. We currently have three Case Manager positions available with our Acute Care team:
- One position is a community-based role with frequent travel throughout Yakima County to provide outreach and support on-site with individuals who are in crisis. The schedule for this role is Monday-Friday 2pm-11pm.
- The other two positions are based in our Crisis Triage Center in Yakima and will not travel. Instead, they'll provide support, monitoring, and resources to individuals placed in stabilization beds or those who present face-to-face at the facility for assistance. They'll also provide support by phone to callers and relay information to the staff who are responding out in the community. Both of these positions have a schedule of four, 10-hour shifts from 8am-7pm; the assigned days of the week will rotate between two schedules and will include working two weekends per month.
Crisis Case Manager duties may include:
- accompanying Crisis Outreach Professionals on community referral calls to gather information and provide direct supportive counseling
- arranging transportation & other services and following up with clients after crisis contacts as assigned
- responding to requests for crisis outreach and assessing for immediate safety, providing appropriate interventions when indicated
- coordinating services between Comprehensive and community providers
- monitoring symptoms & medication response and encouraging clients to practice illness self-management skills
- coordinating with clients’ treatment teams to ensure all discharge and/or referral needs are being met
- completing all documentation in a respectful, client-centered manner
Qualifications:
- Bachelor's degree in social work, psychology, or related field
- Must be eligible for registration as Agency Affiliated Counselor
- Must have valid driver's license and vehicle liability insurance
