TITLE: Community Health Representative (Arlee)
LOCATION: Tribal Health Department
SALARY: $16.46 - $18.92 per hour, plus benefits
CLOSING DATE: Thursday, March 28, 2024 at 5:30 p.m. (MST)
SPECIAL CONDITIONS:
The Community Health Representative is a core member of the collaborative care team, including the
patients’ medical provider, behavioral health provider, as well as the larger primary care team. The
Community Health Representative is responsible for assisting in supporting and actively participating
indirect patient care in the following ways:
➢ Explains and educates patient/family on services offered by Tribal Health including Tribal
Care Coordination.
➢ Accompanies care managers and/or other members of the care team on home visits, as
applicable.
➢ Makes independent home visits for the purpose of promoting patient communication with
the care team.
➢ Acts as an advocate by assisting patients to navigate health and social service systems and
resources to include but not limited to: accompanying patients to health-related
appointments, coordination of health appointments, building rapport and trust with
patient/family.
➢ Provides health promotion education as directed by the care managers.
➢ Assists patients in coordinating transportation to health-related appointments.
➢ Makes referral to community resources (ex. Food banks, meal on wheels, etc.)
➢ Prepares and sends patient communication, as directed.
➢ Ensures patient demographic and other pertinent information is up-to-date in the current
electronic health record.
➢ Is mindful of traditional healing methods that may be used by the patient and communicated
back to the Care Teams.
➢ Work schedule is not always reflective of a typical 8hr day and will require staggering
scheduling in order to better serve patient care needs.
➢ Documents patient encounters in an appropriate electronic health record.
Under the direction and supervision of a registered nurse, the Community Health Representative
may obtain vital signs, per protocol.
The Community Health Representative informs the care teams on the factors that affect the health
of the communities in which they serve, assists the team in assessing the health status of the
community, and identifies assets and resources that can be used for improving the health of the
community.
The Community Health Representative builds and maintains a respectful working relationship with
the Kootenai Culture Committee, Salish and Pend d’ Oreille Culture Committee, Elders
Committees, and other CSKT departments, programs, and resources.
The Community Health Representative promotes health and wellness, being mindful of traditional
methods, including the planning and implementation of community-based and reservation-wide
activities.
The Community Health Representative contributes to the development of Tribal Care Coordination
Division goals and objectives by:
- Engaging community members in improving their own health.
- Providing input for developing, implementing, evaluating, and improving policies,
programs, and services.
Perform other duties as assigned.
MINIMUM RECRUITING QUALIFICATIONS AS REFLECTED ON TRIBAL
EMPLOYMENT APPLICATION:
- High School Diploma or equivalent
- A minimum of 2-years’ experience working in the health care setting OR
- Proof of a Certified Nursing Assistant course completion with a minimum of 1-year work as a
- Valid and unrestricted driver’s license.
- Heartsaver CPR or BLS within six months of hire
- Completion of the IHS CHR course within 3 months
- Experience with an electronic health record preferred.

3. Copy of valid driver’s license.
4. Proof of enrollment from a federally recognized Tribe if not from CSKT.
5. If claiming veteran’s preference, a copy of the DD214 must be submitted.
