Position Summary:
The Intake Coordinator plays a critical administrative and customer service role within the home health agency, serving as the first point of contact for new patients, referral sources, and physicians. This position ensures accurate and timely intake and processing of patient referrals and coordination of services with clinical and office staff.
Key Responsibilities:
Receive and process referrals from hospitals, physicians, case managers, and other referral sources.
Verify patient insurance eligibility, benefits, and authorization requirements.
Accurately enter patient information into the electronic health record (EHR) system.
Communicate with physicians to obtain necessary orders and documentation for patient admission.
Coordinate start-of-care schedules with clinical staff, ensuring timely patient visits.
Maintain organized records and documentation in compliance with agency policies and regulatory standards.
Communicate with patients and families to collect intake information and explain services.
Collaborate with the billing department to ensure payer requirements are met before admission.
Track and follow up on pending referrals and documentation.
Assist with general office duties such as answering phones, filing, faxing, and data entry.
Qualifications:
High school diploma or GED required; associate degree or higher preferred.
1–2 years of experience in a healthcare office setting, preferably home health or hospice.
Familiarity with Medicare, Medicaid, and private insurance requirements.
Strong computer skills and experience with EHR systems (e.g., WellSky, Homecare Homebase, Kinnser, or similar).
Excellent communication and interpersonal skills.
Strong attention to detail, organization, and time management.
Ability to work in a fast-paced environment with competing priorities.
Preferred Skills:
Knowledge of home health regulations and documentation requirements (CMS guidelines).
Bilingual abilities (e.g., Spanish/English) a plus.
Prior experience coordinating care between patients, clinicians, and insurers.
Working Conditions:
Office-based position with potential for remote or hybrid flexibility.
May require occasional overtime based on referral volume or deadlines.
