Consumer Direct Care Network complies with the OSHA COVID-19 Vaccine and Testing Emergency Temporary Standard. As a condition of employment, individuals will be required to 1) submit either proof of their fully vaccinated status or 2) submit weekly test results. Employees may be approved for a vaccine exemption based on a medical condition or sincerely-held religious belief and abide by approved accommodations, however, weekly testing will be required. Your vaccination status will be verified during new employee onboarding if you are selected for hire and will be kept confidential.

JOB SUMMARY
Service Coordinators maintain and coordinate a caseload of caregivers who are providing services for Clients enrolled in Medicaid programs. These duties include assisting with day-to-day functions as well as answering questions related to the provision of these services, while following company policy, Federal/State regulations, program rules, and contract requirements.
JOB DUTIES
- Assist IPs, Clients, and stakeholders through multiple communication channels (e.g. phone, email, in-person, etc.)
- Demonstrate excellent customer service skills
- Assist caregivers in navigating services, web portal, and EVV systems as needed
- Ability to demonstrate dependability and to meet position requirements with little direct oversight
- Effective written and verbal communicator
- Demonstrate effective problem solving and decision-making skills
- Exhibit computer proficiency
- Bilingual preferred in various locations
- Some travel in region may be required up to 20%
- Other duties as assigned
QUALIFICATIONS
- Bachelor’s degree preferred, but not required
- 1 year of previous experience working with individuals with disabilities or the elderly preferred
- Combination of education and experience
- Reliable transportation required
- This position may be hybrid between an office and home office. Hybrid employees must provide their own reliable home Internet.
The incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed to perform duties. The noise level in the work environment is typical of that of an office. Incumbent may encounter frequent interruptions throughout the workday. The employee is regularly required to sit, talk, or hear; frequently required to use repetitive hand motion, handle or feel, and to stand, walk, reach, bend or lift up to 20 pounds.
