Job Title: Patient Service Representative

The Patient Service Representative is responsible for creating a welcoming home like environment; an impeccable telephonic patient experience; ensuring medical records are received, uploaded and extracted to individual patient charts; and any administrative support needed.
The Patient Service Representative must be eager to learn, customer-service driven, a self-starter, engaging with patients, staff and vendors. ConvenientMD Primary Care is a fast paced environment and evolving and as a startup business model. It is essential to ConvenientMD Primary Care, that the Patient Service Representative can present the willingness to be flexible and create resolutions while supporting business efforts and solutions where and when needed.
Job Responsibilities:
1. Customer Service & Value Based Care:
• Creates a warm and welcoming experience for patients. Establishes professional relationships with patients and staff.
• Educates patient of the primary care value based care model and shared responsibility.
• Manages all in-bound calls, out-bound calls and scheduling of appointments.
• Register patients by gathering necessary information from patient to build a new and or updated patient account in the Electronic Medical Record.
• Provides back up support to the Patient Advocate team as needed.
• Team player with the Population Health staff by volunteering to help where needed.
2. Managing Administrative Duties:
• Perform various administrative duties such as photocopying forms, creating reports, faxing or mailing forms to patients, providers, filing charts and maintaining patient trackers.
• Has working knowledge of EMR and is able to schedule or cancel appointments; add telephone notes to document patient encounters and task MA’s and other team member’s as needed.
• Continuous learning and development of EMR and how to better support patient care and clinic staff.
3. Medical Records:
• Ensures patients have medical records in their medical chart and request records from previous provider group/s.
• Electronically records, tracks, files incoming medical records.
• Able to supports MA’s and Population Health team in obtaining medical records as needed.
Qualifications:
1. Skilled in all of Microsoft Office Suite.
2. High school diploma or equivalent is required.
3. Ability to work in a fast-paced environment and effectively learn new knowledge and skills.
4. Customer service or call center experience preferred.
Physical Requirements:
1. Requires the ability to stand and sit for hours at a time. Some bending and stooping is required.
2. Must be able to use manual dexterity in relation to clinical requirements.
3. Must be able to lift up to 50lbs.
