Short Description
The Nurse Navigator II is an integral part of the Oncology Multidisciplinary Team.
The NN II supports, educates, identifies barriers to care and assists patients and families adjust to a new cancer diagnosis. This role helps patients navigate the complexity of the healthcare system and contributes to improved patient outcomes and patient and provider satisfaction.
Connects with newly diagnosed patients prior to first apt and at first appointment with oncology providerFacilitates the Multidisciplinary Care at MDA Cancer Center at Cooper University Health CareSupports, educates and removes barriers to ensure timely specialized careAssesses patients for Clinic Research TrialsCoordinates Disease Specific Tumor Board Complies with all system and departmental policies and proceduresMaintains active and current professional credentials
