Home Health Registered Nurse

Key Responsibilities:
- Patient Care: Conduct admission, case management, and follow-up skilled nursing visits for home health patients.
- Ongoing Support: Provide continuous care and case management, coordinating with the Clinical Manager for necessary follow-ups.
- Treatment Planning: Collaborate with physicians to develop and revise the initial and ongoing treatment plans based on assessments and physician orders.
- Hands-On Care: Deliver hands-on nursing care, evaluate care plans, and educate patients in accordance with physician instructions and under the Clinical Manager’s supervision.
- Coordination: Work with a multidisciplinary team, including Physical Therapists, Occupational Therapists, Home Health Aides, and external providers to ensure comprehensive care.
- Communication: Regularly update physicians and the Clinical Manager on patient conditions and progress.
- Patient and Family Collaboration: Assist patients and their families in resolving needs and problems, ensuring optimal outcomes.
- Supervision: Oversee and supervise Home Health Aides and Personal Care Workers, providing clear instructions and ensuring proper documentation.
- Discharge Planning: Prepare and complete clinical documentation for patient discharge in consultation with the physician and Clinical Manager.
- Documentation: Maintain accurate medical records for all patients, including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan adjustments.

Required Experience and Skills:
- Diploma, Associate, or Bachelor’s Degree in Nursing
- Home Health experience is a plus
- Current and unrestricted Registered Nurse licensure in the state of Montana
- Current CPR certification
- Excellent organizational and communication skills
- Valid driver’s license, auto insurance, and reliable transportation
