Part-time, nonexempt : 20 - 24 hours per week
 

Coordinated Care Services Inc (CCSI) seeks a talented Violence Prevention Caseworker to contribute their expertise in providing essential business services to our provider organizations and customers.  As the Violence Prevention Caseworker, you will focus on delivering comprehensive case management services aligned with the objectives outlined in the safety plan. This role ensures the fulfillment of clients' fundamental needs and maintains regular communication to monitor the implementation of their safety plan, while also aiding them in progressing towards personal sustainability.


CCSI is partnering with Rise-Up Rochester, Inc. to hire a Violence Prevention Caseworker This position is employed by CCSI and working on behalf of Rise-Up Rochester, Inc.


In this role, you will be responsible for

  • Conducting assessments of individuals affected by violence to identify their unique needs and challenges.
  • Developing and implementing personalized case plans to address immediate safety concerns and long-term well-being.
  • Collaborating with community resources and agencies to secure safe and suitable housing options for clients.
  • Advocating for clients in securing temporary and permanent housing solutions, and addressing homelessness or unsafe living conditions.
  • Conducting safety checks when temporary housing is secured.
  • Coordinating with outreach workers to conduct safety checks in hotels to ensure clients are not damaging the property.
  • Working closely with hotel management to address any concerns and maintain positive relationships.
  • Developing and implementing a comprehensive plan to ensure that the basic needs of all clients are met.
  • Providing 20-30 hours per week in direct support related to crisis and response.
  • Guiding tenants in problem-solving, conflict resolution, and assistance with housing needs.
  • Offering case management to victims, their families, and neighborhoods directly impacted by a violent incident.\
  • Maintaining the privacy and security of information provided by participants.
  • Ensuring complete confidentiality of participant encounters, referrals, and case management records.
  • Collaborating with other organizations engaged in community outreach to enhance collective efforts.
  • Participating in community events and initiatives aimed at violence prevention and support.
  • Other duties as assigned

Requirements

  

  • Minimum of Associate degree plus 5 years of work experience in crisis intervention and/or violence prevention  (see CCSI's equivalency chart)   

Additional Requirements

  • Satisfactory completion of all required background screenings.
  • Valid NYS driving license and insurance.
  • General comprehension of DHS system.
