POSITION/BUSINESS TITLE: Mental Health Specialist, Court Liaison 

PROGRAM/DEPARTMENT: Monroe County Office of Mental Health (MCOMH) Socio-Legal

Transition Management program

REPORTS TO: Forensic Program Supervisor  

CCSI SERVICE LINE: Program Management 

LOCATION: Rochester, NY

ABOUT THE POSITION: CCSI is contracted by Monroe County Office of Mental Health to provide program management services, including specialized staffing. The Transition Case Specialist works with incarcerated or otherwise legally involved individuals who are diagnosed with mental illness through their transition into treatment and services in the community.


SPECIFIC RESPONSIBILITIES INCLUDE:

Service System Performance Management:

  • Assess discharge planning needs of individuals as referred by Socio-Legal Center staff, jail personnel, and the court system.
  • Interact with other personnel as needed for release planning.
  • Prepare report as required by Monroe County and NYS OMH.
  • Work cooperatively with Department of Human Services staff to ensure Medicaid applications are filed in timely manner.
  • Work cooperatively providing reports to/in Mental Health Court.
  • Undergo Medication Grant application process for those requiring coverage until Medicaid coverage is activated.
  • When indicated, accompany clients to outpatient appointments, as personal safety allows.
  • Assist clients with obtaining basic needs upon release from jail (e.g., clothing, food, shelter).
  • Record progress notes in comprehensive, timely manner.

System-wide Service Planning:

  • Implement and assist with development of policy/procedures to ensure that clients’ treatment needs are met.
  • Communicate effectively with providers/criminal justice personnel and families to assist clients with developing and following discharge plan.
  • Monitor response of providers and assist with development and implementation of performance improvement activities.

General Responsibilities:

  • Use team approach in working with Socio-Legal Center staff and the community.
  • Incorporate a recognition of diverse clientele into daily activities.

Remain culturally responsive to population being served in development, design, monitoring, evaluation and/or implementation of programs and services.

Requirements

EDUCATION AND EXPERIENCE:

  • BSW, BS, BA in Mental Health, Social Work, or Human Services related area plus 2 years’ experience in mental health field, OR Associates degree in Criminal Justice or Human Services related Field plus 3 years’ experience, OR equivalent education and work experience (see CCSI's equivalency chart).

ADDITIONAL REQUIREMENTS:

  • Ability to satisfactorily complete required background screenings (criminal, SCR, driving, and Monroe County Jail).
  • Valid Class D New York State Driver License and access to reliable transportation to meet transportation requirements of position
