Responsibilities:
- Provide counseling services to individuals, families
- Conduct assessments and develop treatment plans
- Collaborate with other healthcare professionals to ensure comprehensive care
- Provides assistance with transplant referrals
- Provides information and assist with referrals to community resources
- Ensure proper patient care by monitoring progress and making necessary adjustments to treatment plans
- Maintain accurate and up-to-date medical records
- Facilitates new patient admissions
Qualifications:
- Master's degree in Social Work
- Valid state licensure as a Social Worker
- Strong interpersonal skills and the ability to build rapport with clients
- Excellent communication skills, both verbal and written
- Ability to work effectively as part of a multidisciplinary team
- Proficiency in medical record documentation
