Description

We are looking for a BHT Outreach Worker to join our team! This role will provide mental health and substance use disorder treatment, linkage to resources, and related recovery support to adults within the jail and justice system. The BHT Outreach Worker will focus on identifying, assessing, and treating individual’s needs to connect individuals with appropriate services. Staff will conduct clinical assessments and screenings to determine the presence of mental health, substance use, or co-occurring disorders, and develop personalized, medically necessary service plans for each individual enrolled in the program. This position will provide follow-up care for individuals participating in medication-assisted treatment (MAT) programs. The Specialist will be responsible for outreach, intake, and ongoing support services, and will serve as a liaison between the program and other community agencies, specifically within the jail and justice population. The BHT Outreach Worker will report directly to the Project Director.

Minimum Qualifications:

  • Bachelors or master’s degree, no experience necessary or
  • Associate degree plus two (2) years behavioral health work experience or
  • High School Diploma or GED plus Peer Support Specialist certification plus three (3) years of behavioral health work experience or
  • High School Diploma or GED plus four (4) years of behavioral health work experience.

Additional Requirements:

  • Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a clean or acceptable driving record. 
  • At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.
  • Applicants must be able to pass a pre-employment drug screen and background.
  • Level 1 Fingerprint Clearance Card 
  • Ability to maintain Jail Clearance

Care and Treatment: Provide client care in a manner that protects client rights recognizes and respects cultural differences and promotes client dignity, independence, individuality, strengths, abilities, privacy and choice.

  1. Perform outreach and engagement to target population, coordinate with outside service providers, and entities.
  2. Assist with the administration of the Government Performance and Results Act (GPRA) survey in accordance with SAMHSA grant requirements.
  3. Provide substance abuse education and treatment support (group and individual) to the adults participating in opiate addiction and MOUD treatment services.
  4. Participate in interdisciplinary staffing for clients with other team members as appropriate.
  5. Provide intake and ongoing assessment of individuals with mental health and substance abuse issues.
  6. Interface as needed with peers and other clinical team members to coordinate client services.
  7. With client’s consent, collaborate with other community resources, referral agencies, and outside agencies to facilitate needed services for clients.
  8. Maintain regular contact with each client’s treatment team, providing written reports as required.
  9. Participate in the evaluation activities for the project.
