Copeland Oaks, a non-profit Continuing Care Retirement Community, is hiring a full and part time Assisted Living- Memory Care LPN Supervisor for our afternoon shift. Our beautiful 250-acre campus includes Independent Living, Catered Living, Assisted Living and Long-Term Care and Rehabilitation services.
Assisted Living residents are able to live in a "nearly independent" apartment like setting while having the reassurance of onsite nursing assistance and services. Our LPN Supervisors provide one on one care while monitoring for any change in status and managing medications to ensure residents receive their required care.
LPN Essential Functions:
- Administration and ordering of medications
- Preparation for transfers to Crandall Medical Center, hospitals, physicians, and clinics' including all documentation such as transfer forms, change of accommodations, and notification of families.
- Performs general nursing functions within scope of license.
- Liaison with other clinics, physician offices, hospital staffs and families, and pharmacies and other departments in Copeland Oaks and Crandall Medical Center.
- Supervises STNAs.

Professional Experience/Educational Requirements:
- Graduate of an accredited school of practical nursing with pharmacology.
- Medical - Surgical Nursing, Home Health, or Long-term care experience
Certification/Licensure:
Current State of Ohio Licensed Practical Nurse required.
