What are the responsibilities and job description for the Discharge Planner position at Cornerstone of Rhinebeck?
Cornerstone of Rhinebeck is a short-term, inpatient chemical dependency treatment program for adults. We are currently seeking Full Time Discharge Planner to work directly with patients.
Monday through Friday 8 : 30AM-5PM
Hours are Flexible
Requirements :
- High School Diploma with two or more years of education and / or experience in chemical dependency counseling
- One to two years’ experience in discharge planning preferred.
- Ability to perform case management tasks with minimal supervision.
- Must be computer literate.
- Prior experience with Electronic Medical Records.
