Responsibilities of the Nurse Care Manager
- The RN works as part of a multidisciplinary Assertive Community Treatment team, effectively interacting with patients, significant others and other treatment team members while maintaining all standards of professional nursing. Coordinate somatic & behavioral health care through communication with internal & external providers
- Collecting vitals, coordinating with internal and external providers, administering medication and providing assertive case management to include appropriate assessment, coordination, linkage, delivery and advocacy of services for all assigned clients. Local travel is required
Qualifications of the Licensed Practical Nurse
- One to two years of experience providing mental health services and/or addictions treatment.
- Active Maryland RN licensure required
- Possession of a valid driver’s license and satisfactory driving record (as determined by HR) required. Must have access to a personal vehicle and be willing to use it for work purposes. Transporting clients in personal vehicle is required.
