PRIOR TO EMPLOYMENT, THE FOLLOWING INFORMATION WILL BE NEEDED FOR CONSIDERATION (YOU CAN OBTAIN, TURN IN, AND COMPLETE THIS INFORMATION AT THE NORTH PUTNAM ADMINISTRATION OFFICE - 300 NORTH WASHINGTON STREET, BAINBRIDGE, IN 46105)
- W-4 Federal and State Withholding Certificates
- Indiana New Hire Reporting Employer Information
- Form I-9
- Racial & Ethnic Data Form
- Copy of your valid Driver’s License or photo ID issued by federal or state government
- Copy of your Social Security Card
- A Copy of your college transcripts
LICENSED APPLICANTS
- Provide a copy of a valid Indiana Teacher’s License
NON-LICENSED APPLICANTS
- Provide a copy of a valid Substitute Teacher’s Permit
If you currently do not hold a valid Substitute Teacher’s Permit, please log on to www.doe.in.gov/educatorlicensing and complete the online Application for Indiana Substitute Teaching Permit. The Application fee is $15 and is payable online by VISA or MasterCard to the State of Indiana. We need a copy of your license before we can finalize your application.
ADDITIONAL REQUIREMENTS
- The minimum education requirement to be a Substitute Teacher is 15 college hours from an accredited institute. The fifteen college hour requirement will be waived if the candidate has a high school diploma and two or more years of verifiable successful service as an instructional assistant, athletic or academic coach, child care worker, 4-H leader, or scout leader.
- Interview with Administration Office Director
- Complete substitute teaching orientation
COMPENSATION
Licensed Teachers and others with a 4-year degree: $85.00 per day
All others: $75.00 per day
