As a PASSPORT Care Manager, you will provide intake, assessment, and ongoing care management services to clients enrolled in the PASSPORT program. You’ll conduct home visits, develop and monitor person-centered care plans, coordinate medical and social services, support caregivers, and ensure individuals receive the services they need to live with dignity and independen
ce.
