What are the responsibilities and job description for the ADRC Professional I/II position at County of Jackson?

Under the direction of the Aging & Disabilities Services Manager, the ADRC Professional provides a wide variety of services including assisting clients with supportive services designed to overcome financial, health, or family problems; conduct individual or family assessments to determine services needed; provide agency services when appropriate; and refer clients for other community services when needed. Offers assistance built on strengths of individuals, families, and the community.  Works collaboratively with internal and external services in order to advocate for consumer needs to assure services are delivered.

 ESSENTIAL DUTIES AND RESPONSIBILITIES 
  • Creates a positive, courteous and customer service based image of the ADRC to customers, co-workers, and the general public.Uses motivational interviewing techniques to clarify opportunities to best meet customer objectives.Strives to meet customer satisfaction of ADRC services
  • Utilizes ADRC tools such as client database, resource database, resource guides, pamphlets, technology, and documents (service agreements, release of information forms, referral forms, etc.) effectively
  • Follows ADRC process for interviewing, documenting, providing service, making referrals, coordinating with service agencies, and following up with customers 
  • Processes new referrals and completes the Functional and Financial Screen to determine eligibility for Family Care services/IRIS (Include, Respect I Self-Direct)
  • Offers options counseling to individuals who are elderly, physically disabled, developmentally disabled, and those who are experiencing a mental illness or substance abuse issue either on a one-to-one or public outreach setting
  • Explains and discusses options with clients regarding programs and services that are available for individuals in the target groups
  • Makes home visits to assess client's needs and finances and offers the client services and/or program options
  • Performs evidenced based programming in community based settings
  • Provides information and assistance to consumers, families, caregivers and other professionals regarding services per Wisconsin Statutes, and/or requirements of the program, state, or federal guidelines
  • Accepts referrals for department/agency services
  • Conducts thorough assessments through interviews, home visits and collateral investigations.Determines the range of services needed by individuals and their families for preventative, remedial or rehabilitative services
  • Explains the scope of the agency's services and discusses the client's rights and responsibilities in relation to the use of such services
  • Determines eligibility for funding of services
  • Provides case planning and case management services; conducts regular case evaluations and develops and updates case plans accordingly
  • Maintains case records, completes necessary documentation, court reports, forms, and data collection in a timely manner and in accordance with program requirements
  • Participates in committees and task forces as assigned as a liaison to various community agencies and organizations
  • Advocates for client needs
  • Maintains confidentiality of client-related information and maintain respectful treatment of clients
  • Testifies in court as necessary or required
  • Maintains skills and competence levels as policies and regulations change by attending department/staff meetings and participating in continuing training as required and appropriate. Follows required state training guidelines pertinent to the duties assigned
  • Assists and provides back up to other workers as necessary or required
  • Performs other duties as assigned
EDUCATION AND EXPERIENCE REQUIREMENTS
  • A Bachelor of Arts or Science degree (preferably in a health or human services related field) or a license to practice as a registered nurse in Wisconsin pursuant to s. 441.06 Stats
  • Minimum of one (1) year of experience  or the equivalent in a health or human service field, working with one or more of the client populations served by the Aging and Disability Resource Center ( elderly or adults with physical or developmental/intellectual disabilities).  Qualifying work experience may be paid or unpaid and may include internships, field placements and volunteer work
CERTIFICATES, LICENSES, REGISTRATIONS
  • Possess and maintain valid Wisconsin Driver's license and have access to transportation suitable to complete necessary work out of the office
  • Must be or become certified a Long Term Care Functional Screener within the first six months of hire
  • Must be or become AIRS certified within the first year of hire

ADRC PROFESSIONAL II

DISTINGUISHING FEATURES OF WORK
This is an agency-trained professional position.  Employees in this class perform relatively the same duties as that of ADRC Professional I but at an advanced level.  They will demonstrate advanced knowledge and skills gained through experience.
 
EXPERIENCE AND TRAINING REQUIREMENTS
Must have received 70 hours of approved training attained after graduation and within the most recent ten years (training must be in compliance with the core training topics required within each division).  Must also have acceptable job performance and 24 months of experience at the Social Worker I or ADRC Professional I level.
 KNOWLEDGE, SKILLS AND ABILITIES REQUIREMENTS
  • Knowledge of the aging process, physical and developmental disabilities, mental health issues and substance abuse, local socio-economic issues and their effect on individuals and families
  • Basic understanding of social systems, culture, diversity, family dynamics, and economic indicators of personal ability to achieve personal needs/outcomes
  • Knowledge of Medicaid, Medicare, Social Security, various other financial assistance programs IRIS and Family Care
  • Knowledge of the philosophy, history and development of social welfare programs
  • Basic knowledge and understanding of human growth and behavior
  • Knowledge of laws, regulations and practices pertaining to federal and state public welfare programs. This knowledge includes a thorough understanding of the functional and financial aspects of eligibility for public programs
  • Knowledge of resources for community welfare and health and ways in which these resources may be used by people in need
  • Basic computer keyboarding skills for entering data relation to clients
  • Ability to relate appropriately to consumers/clients, family members and service providers
  • Ability to conduct thorough individualized needs assessments
  • Ability to work with diverse populations
  • Ability to relate to people in an unprejudiced and understanding manner with concerns for their circumstances and feelings
  • Ability to communicate clearly and effectively orally and in written form
  • Ability to plan and organize work to most effectively achieve program objectives
  • Ability to understand and follow oral and written instructions
  • Ability to participate in and appropriately use available supervision
  • Ability to participate meaningfully in training provided by the agency and other resources
  • Ability to read and interpret documents, and laws in the English language
  • Ability to listen to and understand information and ideas in spoken and written format so individuals may effectively understand and convey communications
  • Ability to participate meaningfully in training provided by the agency, State Department, the University of Wisconsin Extension , and other resources
  • Knowledge of Chapter 51, 54 and 55of the Wisconsin Statutes and the system in place that supports these laws
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
  • Ability to compute rate, ratio, and percent
  • Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form
  • Inductive and deductive reasoning – ability to combine separate pieces of information, or specific answers to problems, to form general rules or conclusions; includes coming up with a logical explanation for why a series of seemingly unrelated events occur together and deciding if an answer makes sense
HIPAA RESPONSIBILITY

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  The employee is regularly required to stand, walk, use hands to finger, handle or feel objects; reach with hands and arms, hear and talk.  The employee may be required to run, sit, stoop, kneel, crouch, crawl, climb, balance, taste and smell.  May seldom be required to lift and/or move up to 50 pounds.  Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
